Ranges Active Living program is primarily responsible for delivering Home and Community Care (HACC) funded Allied Health Services to the local community. These include: dietetics, occupational therapy, physiotherapy and podiatry. Ranges' also provides a HACC funded social support program and one Planned Activity Group.

The Active Living program provides a range of group programs to meet the needs of the target population. These include: better health self management (for chronic conditions), hydrotherapy groups, water exercise groups and an exercise for balance group.

As the name implies, the Active Living program aims to promote a positive, active and healthy outlook in all areas of our work.

Team Purpose: To support senior members of the community and people with a disability to strengthen their capacity to improve their own health

Target Population: Older people and people with a disability who reside in Ranges CHS catchment area and are eligible for the Home and Community Care (HACC) Program.

Team Goals:
1. To provide direct care services with consumer input that are relevant and accessible to our target group.
2. To ensure those most in need and those experiencing disadvantage receive priority services.
3. To facilitate and raise awareness of health promoting programs to our target group.

Groups and programs include:

Water Exercise Water Exercise classes are run at venues across Knoxbrooke, Monbulk and Lilydale.

Interlink The Interlink program is run from Kallista Community House and provides planned activities for adults with
Intellectual disabilities.


Hospital Admission Risk Program (HARP) The Hospital Demand Management program is a statewide strategy of the Department of Human Services underpinned by a vision of better health for Victorians through preventing the avoidable use of hospitals. Its purpose is to build capacity to meet the growth in demand for hospital services and includes the Hospital Admission Risk Program (HARP), which has a particular emphasis on community-based preventative initiatives.
