Physical Activity
Participation in the Primary Care Partnership for improving access to physical activity has been a priority for Ranges over the past year.

Two programs have been extensively evaluated and support the evidence base for physical activity program development.

'No Falls' Program
The 'No Falls' program at Kirkbrae Retirement Village was introduced to this community setting by Ranges
Physiotherapy staff and was actively supported by the management at the village.

The success of the program can be attributed to the development of a collaborative partnership between Ranges CHS and Kirkbrae.

Currently it is co-ordinated through management and is fully resourced through a designated no falls leader, physiotherapist on site and volunteers -therefore making this a sustainable approach.

The residents of the independent living units are very engaged in the program and obtain both physical and social benefits from their involvement in the sessions.

Ranges CHS presented the Kirkbrae no falls evaluation at the PCP Inter Active Forum where many agencies were able to share their experiences from the field.

A description of the 'Safe Moves'
program has been entered on the Quality
Improvement Program Planning System. The
evaluation indicated that 96% of participants had improved knowledge of falls and improved balance.

The Older But Stronger program
The Older But Stronger program aims to improve the access to, and participation in, strength training fitness programs across Ranges CHS
catchment to isolated, frail older people and others with special needs, has been in existence for five years.

An extensive review was initiated and findings are available on the Quality Improvement Program Planning System.

This review was also presented at
the PCP Inter Active Partnerships Forum. A model of practice has been recommended involving the investigation of employing a Fitness Leader with a
Certificate IV in Aged Care.
