Hospital Admissions Risk Program


The Hospital Admissions Risk Program (HARP) is a partnership with Eastern Health. Ranges Community Health HARP services include physiotherapy, occupational therapy and short term psychosocial care coordination.

 

Who is this for?

HARP services are for people in our community who are frequent presenters to hospital emergency departments or are at risk of presenting to emergency.

Short term psychosocial care coordination services are for people with frequent hospital presentations and two or more of the following:

  • Mental health issues such as depression and anxiety
  • Drug and alcohol related concerns
  • Problems with homelessness or risk of homelessness
  • Social isolation or interpersonal issues
  • Acquired Brain Injury
  • Difficulty with self management
  • Difficulty linking to community based services

Clients are not suitable for Psychosocial Care Coordination if they are case managed by other services, pose a threat to workers’ safety or can be effectively assisted by other services.

 

How can HARP assist you?

The aim of the program is to assist our clients to receive effective, timely and appropriate care in the community. Where possible, we work with clients to reduce the frequency of admissions to hospital, maintaining better quality of life in the community.

Care coordination may include:

  • Comprehensive assessment
  • Development of an agreed care plan with the client
  • Consultation with other treatment providers involved in the client’s care to promote a coordinated treatment plan and enhance communication between services
  • Provision of short term support and assistance with immediate needs
  • Referrals to other specialist services
  • Advocacy for the client
  • Assistance to develop better self management
  • Secondary consultation to other services
  • Capacity to meet with clients in their home or in the community
