Who is this for?
The Home and Community Care (HACC) Program provides a range of support services to frail older people and people with disabilities who are experiencing difficulties in managing daily tasks but who wish to continue living at home. The Program also supports their carers and families.
How can HACC Services assist you?
- Allied health- provides advice and treatment in relation to problems you may have with movement (Occupational and Physiotherapy), foot care/walking (Podiatry) and diet (Dietetics).
- Interlink- a planned activity group for people with intellectual disabilities run from Kallista Community House. It offers a wide range of experiences and creative opportunities and aims to promote self help and foster independent living and social skills.
- Water exercise- take the weight off your joints and build up strength in a fun, supportive group whilst exercising in water. Classes are run at venues in Knoxbrooke, Monbulk and Lilydale. See Term Program for further details.
- Social Support- a resource and support service for older isolated people and people with disabilities. The program offers support, advice and links to appropriate community based networks and services to assist people to live independently in the community. In addition the program offers friendship group outings and a stroke support group.
The HACC team are currently working towards implementing a quality improvement initiative, the Active Service model (ASM) in partnership with local agencies. The ASM focuses on promoting person centred care, encourages independence and social inclusion and supports partnerships between all health providers. The program puts emphasis on timely interventions that prioritise restorative care to maintain your capacity to live independently.
