EARLY INTERVENTION in CHRONIC DISEASE


The Early Intervention in Chronic Disease program is a recently established service for people with chronic diseases in particular focusing on those with type 2 diabetes.

Making healthy lifestyle changes and being actively involved in your own health decisions will result in you feeling fitter, healthier and in control.

With the support from our specialist health care team and a Key Worker we will assist you to develop new skills to manage your condition.

 

Who is this for?

  • Are you - aged 45 plus?
  • Are you overweight?
  • Do you have high blood pressure (on medication)?
  • Do you have high blood glucose (impaired glucose tolerance)?
  • Are you Aboriginal or Torres Strait Islander over 35 years?
  • Do you’ve a history of gestational diabetes (during pregnancy)?
  • Do you have a family history of type 2 diabetes?

Answering yes to any of these questions may indicate an increased risk of developing type 2 diabetes.

 

How Can the Early Intervention in Chronic Disease Program Assist You?

A Key Worker will be allocated to you to assist you through the program and as a contact person. The Key Worker will arrange for you to access members of the specialist team of health care professionals, for example Diabetes Educator, Dietitian, Podiatrist, Counsellor and will link with your GP.

As you achieve your goals you will feel in greater control of your own well being and able to make informed decisions and healthier choices.
